HLA-B*51 is considered the strongest genetic risk factor for Behçet’s disease (BD), particularly in populations from regions along the ancient Silk Road, including the Middle East, East Asia, and the Mediterranean. Studies have shown that individuals with the HLA-B*51 allele are more likely to develop BD compared to those without this allele. Behçet’s disease can manifest with recurrent oral and genital ulcers, uveitis, skin lesions, and arthritis. The presence of HLA-B*51 can aid in the diagnosis, especially in patients presenting with these symptoms in endemic regions. Understanding the presence of the HLA-B*51 allele in patients can help in the diagnosis, management, and potentially in the prediction of disease course in Behçet’s disease. However, it is important to note that not all individuals with HLA-B*51 will develop BD, and the allele alone is not sufficient to cause the disease. While the presence of the HLA-B51 allele is strongly associated with Behçet’s disease, it is not diagnostic by itself. Genetic testing for HLA-B51 may be considered in patients with a high clinical suspicion of BD, particularly in regions with a high prevalence of the disease.
